Now Available! For $7.50 + tax, have all of your pets information on one convenient card and key-chain tag! Personalize the cards with a photo of your choice! Cards include the name and contact information of our veterinary clinic as well as: your contact information, your pets vaccination history and due dates as well as key information about your pets identification. A great card to have on hand for boarding and grooming facilities and to facilitate check in at your vets office or emergency clinic. Although not manufactured for this purpose, the key-chain tag can also be used on your pets collar.
*Please note that these cards are not sufficient for border crossings, however, they may accept them should you misplace your signed Rabies vaccination certificate
